Tax Exemption
Certificate
Agreement
of Guarantee *If you have been in business
less than 3 years.
Fax the completed forms and a copy of your Resale
Certificate to 888-635-7479 or email to credit@imagestar.com.
If you plan to use a credit card while your 30 day terms
are being reviewed, please complete a Credit Card Authorization form.
